To request a Replacement Testamur (Name Change), you must first update your personal details in the system.
Students enrolled from 2008 onwards: please request to update your details via the Change of personal details form in AskMQ.
Students enrolled prior to 2008: please submit your certified name change documents via email to graduation@mq.edu.au
Certified copies need to be supplied to update your name.
To process these changes, we require one of the following forms of supporting evidence:
Once completed please provide your Ask MQ reference number in the relevant field below.
If you have the original testamur that you want replaced, this will need to be returned before we can issue the replacement. Please bring/mail this to the address:

A Statutory Declaration will need to be provided if your testamur has been Lost/Destroyed or Not Received.
In addition to your Statutory Declaration, please provide a valid copy (i.e. not expired) of identification that shows your signature (Driver’s Licence, Passport, etc.) You request WILL NOT be processed until you provide this.
Please scan a copy of these required documents to graduation@mq.edu.au. NB: The 15 day processing time starts from the date that all necessary documents are submitted successfully.
