Graduation Replacement Testamur with Formal Name Change - Pick Up

Please complete the following details

Note: replacement testamurs not available for students graduating from SKTC, NSTC or IECS, SCAE prior to 1990

No replacement testamur (name change) will be issued without the return of an original statutory declaration stating that the original testamur has been lost, stolen or destroyed OR the original testamur. You must also submit a specimen of your signature as it appears on a legitimate form of personal identification (eg. passport, etc) which must be presented at the time of collection. As your name is to be changed, please also provide proof to support this change (eg. Change of name certificate, passport). Your request WILL NOT be processed until you provide this.

Before completing this request, I acknowledge that I have:

1. An original statutory declaration or original testamur

2. A legitimate form of personal identification

3. A legitimate form of proof for your name change

Please also make sure you have sent above the documents to the Graduation Team at Level 4, 18 Wally’s Walk, Macquarie University, 2109.
