Graduation Replacement Testamur - Delivery

Please complete the following details

Note: replacement testamurs not available for students graduating from SKTC, NSTC or IECS, SCAE prior to 1990

No replacement testamur will be issued without the return of an original statutory declaration stating that the original testamur has been lost, stolen or destroyed or a damaged testamur. You must also submit a specimen of your signature as it appears on a legitimate form of personal identification (eg. passport, etc) which must be presented at the time of collection. Your request WILL NOT be processed until you provide this.

Before completing this request, I acknowledge that I have:

1. An original statutory declaration or damaged testamur

2. A legitimate form of personal identification

Please also make sure you have sent above the documents to the Graduation Team at Level 4, 18 Wally’s Walk, Macquarie University, 2109.
